Planning ahead for your healthcare is a gift you can give to yourself and those you love. Join us for a presentation on Advanced Directives. Then next week reserve your spot for individual sessions where you’ll meet with a trained advance healthcare planning volunteer from Hospice of Santa Cruz County to complete your form. You can even get your Advance Directive witnessed to make it a legal document.
